CHS Athletics has a continuing contract with the Center for Orthopedic Surgery & Sports Medicine (formerly Illini Sports Medicine) in Belleville. This contract provides the school with a team physician, Dr Janiece Stewart. The agreement also allows for pre-participation physical exams.
Monday, August 11, the physicians of COSSM will be providing sports physicals at a discounted rate to all CHS athletes. Exams will be given from 6-8:30 p.m. at their Belleville office. All athletes must have a signed parental consent form at the time of the exam. Physicals will be $20 with a portion of that fee being donated to the Athletic Department. (Please note, this physical does NOT meet school requirements for incoming freshmen.) The required consent form can be accessed through the link above.
